"Assistive Technology is a HCBS (Home and Community Based Service) that provides funding for Environmental Modification (E-mod). "E-mods" are adaptations to an individual's environment that allow for more independence or addresses issues related to health and safety. Some examples are ramps, lifts, bathroom and kitchen modifications, fences, safety devices for individuals who are blind or deaf, and widened doorways and hallways."

"This Home and Community Based Service (HCBS) provides primary caregivers of people with disabilities relief by allowing them to hire an individual or individuals of their choice to provide care in their absence. People are awarded a number of hours per year through Office for People with Developmental Disabilities (OPWDD). "

"One time and ongoing funds can be available for people, perhaps for the first time, who need additional resources to afford safe and decent housing. Funds can be used to purchase new or used items to set up a household or to pay toward the rent. "

"Formerly known as Outdoor Recreation, the program has recently expanded to provide other opportunities for enjoyment outside of outdoor recreation activities. The purpose of North Country Recreation is to not only offer people the opportunity to enjoy outdoor activities, but to also enjoy other activities as well, such as crafts, video production, games, online chats, and so much more! This is a program for people with disabilities, including those with more involved physical disabilities who may require assistance."

" Case manager can be available to assist individuals in enrolling in programs and services. Both short and long term follow along is available. In order to receive Family Support Case Management: A person must have a documented diagnosis of a developmental disability A person's needs for assistance must be short or long term Medicaid enrollment is not required"

"Cerebral Palsy Association of the North Country offers "Kids On the Block" and "Making Friends" presentations to kindergarten and 3rd/4th graders throughout schools in 6 counties. These FREE shows provide public education to children to help them learn to understand and accept people with developmental disabilities. The age appropriate content captures the attention of young people and the message is clear: "We are all alike"!"

"A case manager can be available to assist individuals in short-term linkage and referral to services available in their communities. To qualify for Home Service Director assistance, a person would have similar needs as someone with a developmental disability."
