"Homeless housing/rental assistance services are offered to residents in Jefferson, Lewis, and St. Lawrence counties to homeless individuals. These services are intended to stabilize homeless individuals/families with intensive case management services and assist with transitioning them to stable, permanent housing or maintaining stable housing (Permanent Supportive Housing only)."

"Focus on individuals in need of more intensive services after frequent or lengthy inpatient mental health treatment. CTI supports individuals with behavioral health needs who have not been able to stay successfully connected with resources surrounding periods of transition. The goals are to reduce the number of hospitalizations and ensure that participants are linked to resources that will ensure their future success as they transition back into the community."

"A clinician is available on-site at the school to work with students and their families. The satellite clinics operate as an extension of the main outpatient clinic (mental health). Students are eligible to receive most services affiliated with the clinic when enrolled in treatment via the satellite clinic. These services include, but are not limited to: individual counseling, family counseling, psychiatric evaluations, ongoing medication management, case management and psychiatric rehabilitation services."

This service is offered to people who live with their family /caregivers or who live independently. Community Habilitation isdesigned to help strengthen skills and to achieve satisfying andrewarding connections and relationships.
