Open to 18 years and older with a primary diagnosis of serious mental illness and experience substantial impairments in maintaning safe housing.

"Homeless housing/rental assistance services are offered to residents in Jefferson, Lewis, and St. Lawrence counties to homeless individuals. These services are intended to stabilize homeless individuals/families with intensive case management services and assist with transitioning them to stable, permanent housing or maintaining stable housing (Permanent Supportive Housing only)."
