" Temporary rental assistance and furnishings combined with case management services for individuals with a primary diagnosis of a serious mental illness or who experiences substantial impairment in functioning. The program’s goal is to aid individuals in securing and maintaining safe, high quality, and long-term affordable housing. The program fosters integration into existing community services such as employment support, mental health treatment, substance use disorder treatment, and transportation assistance."

"Supportive housing is a person-centered housing model to assist individuals with safe and affordable housing options and to increase housing stability. An individual is linked with a Housing Case Manager who helps them address barriers and identify additional solutions and supports."

Open to 18 years and older with a primary diagnosis of serious mental illness and experience substantial impairments in maintaining safe housing.
