Reintegration Residential Programs for Youth

"The Farm offers a holistic rehabilitative environment for young men ages 16 to 20 with a substance abuse diagnosis. The program is individualized to meet each resident’s diverse needs and offers a variable length of stay up to 12 months with the average of 6 to 8 months.

All services onsite with the exception of specialty services that require a referral. The program provides individual, group, and family therapy as well as equine assisted therapy. Vocational and educational services are provided onsite and patients are able to participate in schooling or obtaining their GED. Physical health and mental health services are accessible within the program."

"THRIVE’s Reintegration Residential Programs for men and women are dedicated to providing services in the early stages of recovery from alcohol and/or chemical dependence.

The program provides a family atmosphere that promotes individuality, healthy decision making and improvement of self-awareness. This is made possible through peer support, individual outpatient treatment planning, and group and individual counseling with a variable length of stay averaging 6-12 months."

Apartment Program

"THRIVE serves 33 adults in multiple apartment locations in Watertown. The Apartment Program serves both men and women, though apartments are not co-ed. Similar to the more intensive residential program, THRIVE’ Apartment Program assists residents in acquiring Rehabilitative skills, including Medication Management, Socialization, and Daily Living Skills. "

"THRIVE provides specialized and restorative services in 24/7 supervised community residences in Clayton, Gouverneur, and Watertown. All residences are co-ed and open to individuals with a serious and persistent mental illness, for ages 18 and over. "

"The program is geared to be family focused, trauma sensitive, and strength based with the goal of keeping the child functioning in the community as naturally as possible while aiding them to successfully return to their home setting. The program provides individualized service planning and skill building services as well as a variety of recreational and educational outings to enhance socialization and independent living skills. "

"Peer services which are non-clinical support services provided by individuals who have lived experience related to substance use and/or mental illness. Our peers help others who are in similar situations to achieve their recovery goals. Peer services can include outreach, engagement, advocacy, coaching, mentoring, recovery planning, and linkage to community resources."

Opioid Treatment Program (OTP)

"Certified and accredited program that provides screenings, assessments, evaluations, and approved medications such as Methadone, Suboxone, Buprenorphine, Naltrexone, Vivitrol, and other Medication Assisted Therapy options to treat individuals with substance use disorders and opioid use disorders, as well as help prevent opioid overdose."

"Five weeks each summer for youngsters with special needs.  The program serves children who reside in Jefferson County, who because of special needs (developmental, mental, physical, or socioemotional), have been reluctant or unable to successfully participate in the recreational programs currently offered.   The program offers the children activities such as arts and crafts, group games, and field trips.  "
