Critical Time Intervention (CTI) Program

"CTI supports individuals with behavioral health needs who have not been able to stay successfully connected with resources surrounding periods of transition. CTI team is an intensive phase-based and time limited service that connects participants to community resources to achieve person-centered goals. The goals are to reduce the number of hospitalizations and ensure that participants are linked to resources that will ensure their future success as they transition back into the community."
