Drop In Social Group for persons who are no longer in school and have a disability.

Services for persons with a disability to help prepare individuals for work (volunteer or paid). Skills taught may include: communication, time-managment, finances, computer skills, office equipment skills, etc.

Services are often offered 1 to 1.

Support and assistance for families to understand and support an adult member's diagnosis.

Mental Health Community Partners Program (MHCP) supports adults and youth aged 12+ with high mental health needs (e.g. serious mental illness or emotional disturbance) through major life transitions (e.g. from treatment to community) with the support of Chosen Supporters.

For youth ages 5-21years of age with a psychiatric diagnosis who are Medicaid eligible

"These are rehabilitative services (skill building) and can be delivered in the home or in the community by a trained provider."

For youth 5 - 18 years of age who qualify who are Medicaid eligible.

"Respite services provide the child and family a break from their daily routine in order to reduce stress and maintain harmony in the home."

Support services that may include: peer support, referrals, advocacy, independent living skills educations, transition assistance.
